It’s often a combination of a Yes passport and a Yes face.

I’m an Australian citizen (with accompanying accent) but with an Indian face.

At multiple border crossings around the world – I’ve had lots of issues and had to provide a lengthy explanation of why I don’t “look” Australian.

One time in Peru – I (along with my wife and two young kids) were actually denied entry, until the customs/immigration officer realised his mistake, and let us through. Turns out he didn’t even look at our passport, just our faces!
